Keep the horse in good condition - if its hunger, thirst and tiredness are above 300, the horse gains 10 health points every hour. If you want, you can also buy health supplements in feed stores, but these are kind of expensive, especially the premium ones (+100 health for 11k).

How high does a horse's health have to be for you to be able to ride him?

It's got to be at least 788. I'm pretty sure of it. I've never ridden a horse with health under that.

Well, for health just make sure the feed, water, and energy stay within a 50%. Also grooming might be a factor if left untreated (I notice more bad events happen to ungroomed horses).
For riding of health I've seen people ride a 200 health horse before, so it could be you can ride it at any health. You can't ride it though if it is thirst, tired, or hungry.
